‘My Eyes Were So Swollen I Was Unable to Even Unseal Them’

Onemanda Preisinger is nervous about her daughter’s upcoming 13th birthday party. It’s not due to the usual causes related to a house full of loud adolescent kids, but since it’s the initial occasion she will reveal her new face to friends and relatives. “Obviously I’m going to inform all guests as they come in, ‘For your information, this is not the way I look,’” states the thirty-year-old property agent from south Florida.

How she looks is, admittedly, a little startling – her face swollen and unnaturally tightened, as though held together by industrial-grade tape. Her altered – and she’s keen to stress, temporary – appearance is the outcome of six cosmetic procedures, such as an endoscopic mid-facelift, performed by a surgeon in Turkey’s Istanbul, last month. “My spouse teared up when he saw me for the first time because I wasn’t able to even open my eyes. That’s how swollen I was,” she explains to me via video call from her home. “I had to tell him: ‘Babe, I’m okay, I’m not in pain. I just look like someone attacked me.’”

Increasing Popularity of Youthful Facial Lifts

According to plastic surgeons, Preisinger is among a rising count of people electing to undergo a facelift in their 20s and 30s – well over a ten years prior to most doctors’ usual candidate age of 40 to 60. (Though many specialists are eager to highlight the industry edict of: “We treat genetics, not age.”) “I have 28-year-olds requesting facelifts,” states London-based aesthetic plastic surgeon Georgios Orfaniotis, a ex-NHS doctor who focuses on the facial area. “It’s a major surgery, and I’m a somewhat worried when I see people choosing it as a personal preference.”

Understanding the Rhytidectomy Surgery

A rhytidectomy, also known as a rhytidectomy, elevates the tissues in the face that start to sag as we age. “Our faces are structured a little like layers of an onion,” explains based in Kent face specialist Marc Pacifico. “Skin and fat on the top, then a stratum of supportive tissue known as the superficial musculoaponeurotic system. Consider the SMAS as the structural foundation of the face. And that’s what causes aging; that is what loosens and sags and drags the dermis down with it.”

Once the preserve of affluent seniors, “filler fatigue” – when overuse of injectable gels expands the face and causes laxity in the skin – combined with the widespread use of weight-loss drugs, low-cost medical tourism, and the advancement of novel, celebrity-endorsed surgical techniques are attracting a different type of customer towards this major operation. Statistics indicate an eight percent rise in facial lifts over the last year in the UK; while a survey revealed that the portion of youthful candidates is growing, with 32% of facelifts now conducted for individuals aged thirty-five to fifty-five.

“Patients are now requesting to appear as the best version of themselves and naturally the techniques are evolving,” comments Orfaniotis. Currently, the trending procedure is the deep plane facelift, a technique evangelised by everyone from Kardashian matriarch Kris Jenner, to fashion designer Marc Jacobs.

Advanced Lifting Method

While a more traditional facelift entails lifting the SMAS, the deep plane facelift loosens the underlying structures underneath, allowing surgeons to reshape the face by moving the underlying layers, and preventing the taut, pulled look occasionally caused by older methods. “We avoid placing tension on the dermis because we’re going below, to the underlying structures,” says prestigious London cosmetic doctor Rajiv Grover, who specialises in the deep plane technique. “It is the deeper tissue that is adjusted. The skin comes with it as a passenger.” Lifting the whole facial structure as a single unit results in a more natural-looking and longer-lasting outcome. It additionally implies that the procedure is no longer being used for the express purpose of youth preservation – or revitalization, as the aesthetics industry describes it – but for “beautification” and contouring, too. “We’re getting numerous requests from clients aged late twenties to mid-thirties for this reason,” notes Grover.

Individual Experience

Preisinger had never planned on getting a facelift – at minimum not at the thirty years old. But years of using hyaluronic acid dermal fillers in an attempt at “facial balancing” implied that by the time she entered her late twenties, she was “botched”. “I didn’t do this because I desired to appear youthful,” she clarifies. “I did it because the filler harmed my face. I wish I had never using it. If I had avoided the filler, I don’t think I would be in this situation currently.”

Whether injectable substances completely disappear has historically been a point of contention in the cosmetic field. “Research have indicated that not only do they rarely completely dissolve,” says Grover, “but they migrate and can obstruct lymphatic channels. A contributing factor to what we call ‘pillow face’ is also the reality that in a sense, the face becomes somewhat fluid-filled because its capacity to drain lymphatic fluid has been diminished.” Although studies into the area is early-stage, warnings on dermal fillers’ potential impact on the lymphatic system have been released, and additional studies announced. An esteemed facial plastic surgeon, commenting on the condition of anonymity, says he would avoid using injectables in a client because of the risks they present. “A lot of doctors avoid discussing this, because the manufacturers who produce injectables can be pretty aggressive.” He’s been told accounts, he adds, of cosmetic doctors facing legal actions after raising their concerns.

Choice and Surgery

For Preisinger, once she started injecting filler, she felt as if she needed to continue replenishing it – particularly when it started to migrate to different areas of her face. She was only twenty-eight when a nearby cosmetic doctor in Florida proposed that a facial and neck surgery might be what she needed to exit the cycle of treatments. After 24 months, she ended up choosing between a list of suggested accommodations in the city, sent to her by the clinic assistant at her doctor’s clinic.

Originally, Preisinger had planned for an eyelid surgery – better known as blepharoplasty – but her surgeon advised her that a facial lift was the right solution for the laxity she was finding in her face. She booked a forehead lift, a mid-facelift and a jawline surgery. The day before her operation, 24 hours post she’d arrived alone in the country, she opted to include a mouth enhancement. “Then he said, ‘We’re going to elevate the lower eyelid puffiness.’ Then he said, ‘I believe if we extract some buccal fat, it will contour your face.’ So I ended up including additional surgeries,” she says. She paid $22,000 (£16,500) for the half-day surgery, including a three-day|
